Cataract surgery with intraocular lens (IOL) implantation in buphthalmic eyes is a surgical challenge. IOL placement and centration can be particularly difficult due to the large size of the eyeball. In this video, we describe the technique for cataract surgery and a new customized IOL for in-the-bag implantation in such eyes.
